Provider Re-Registration

Re-registration is only applicable if you are a current user and/or administrator of the Access UCare (ePlan) site. If you are a current user, please verify your username by typing it in "Username" box below and click "Continue." If you are not a current user and would like to register a new administrator account please click here.

TIP: After verifying your username, you will be required to enter your NPI/UMPI number(enter NPI/UMPI as numbers only; dashes and alpha characters are invalid) OR your UCare Provider ID number. The remaining fields ARE REQUIRED.

TIP: Each account must have a unique e-mail address.
